Complete this form to have a program assessed as a promising program or emerging program. First, make sure you have read our guidelines on submitting a program, and have addressed all the assessment criteria in your submission.
Only facilitating partners can make submissions. You must first discuss your intention to submit a program for assessment with your Funding Arrangement Manager.
If you are submitting on behalf of a community partner, include their contact details under "relevant information". We will contact them if we need more information about the program.
