Sample top up: Labelled questionnaire

A short survey was used for the sample top up recruitment as well as for the retention study (those recruited in 2022 only). The questionnaire included: Background information, Health and Wellbeing, Substance Use, Mental Health, Relationships, Health Service Use, and Income, Work and Education questions. The participants in this dataset have been empanelled and will form part of the future Wave 5 dataset.
