Wave 4 questionnaire

For Wave 4, as all respondents were over the age of 18, only one version of the questionnaire was administered. However, a shorter version of the questionnaire was available for those who were less engaged with the study (e.g. those not completing the survey after many weeks in field). Most respondents completed the questionnaire online, while a small portion completed the survey via telephone.
